INDEMNITY FORM # (1) ONE

THIS FORM IS TO BE SIGNED BY THE PARENT/LEGAL GUARDIAN OF A CHILD USING THE CRESTED CRANE ACADEMY, SWIMMING POOL OR OTHER SPORTING FACILITIES, FIELD TRIPS AND OR ANY OTHER RELATED ACTIVITIES PROVIDED BY THE SCHOOL.

Warranting that I am an adult of full Legal status and the Father/Mother/Legal Guardian of the child/ren attending school at CRESTED CRANE ACADEMY namely;

Do hereby enter into this AGREEMENT or INDEMNITY made in favour of CRESTED CRANE ACADEMY on behalf of myself and my child/ren unequivocally and of my own free will.

I am aware that the activities involving the use of the school swimming pool and other sporting facilities by my child/ren involves inherent risks of illness, injury , death or loss and or damage of property by or to any student through the negligence of others ,forces or nature, or other causes known and unknown.

I RECOGNISE that such risks may be present at any time that my child/ren enter/s upon the school premises and or participate/s in the school activities and I hereby  acknowledge that CRESTED CRANE ACADEMY shall indemnified from any loss due to death/injury or other demise to my child/ren.  The indemnity extends to damage to loss to property or life which may result during school time whether on academic or extramural activities or during passage to or from venue of any such school activity, including being on our School buses, and on field trips.

IMAGES/PHOTOS

I accept that CRESTED CRANE ACADEMY shall have the right to use my child’s photos and other information obtained for advertising purposes on both print and social media without consulting me.

This indemnity shall however, not be understood to enable the school to derogate from any of its responsibilities to ensure the well-being of its students.

DAMAGE TO PROPERTY/STATIONERY

Wilful damage to school property including books and stationery by a student shall be billed to the Parent/Legal Guardian.

LIABILITY OF FEES

I, the APPLICANT, being the Parent/Legal Guardian of the above named pupil/students undertake to pay unconditionally all fees, charges and surcharges payable to the school as and when they as all fall due for payment for any period for which s/he is a registered student of the school.

STUDENT TRANSFERS

Further I understand and agree that should I decide to withdraw the above named child form school I will be obliged to hand to the School \Principal one full term’s notice in writing or in lieu thereof to pay one full term’s fees instead.

School reports, testimonials and a letter of transfer will not be issued until all liabilities have been settled with the school.

Further, I agree that should legal action be instituted against me for failure to abide by the liability for fees, all related charges, costs etc shall be borne by me.

REPORTING

I further promise that my child/ren will report to school on the first day of the opening of each term and that I will work together with the Management of CRESTED CRANE ACADMEY.

NOTE:   THE FOLLOWING SHOULD ACCOMPANY THIS APPPLICATION

  • The completed Medical Form/ Copy of Under Five Card
  • The last Report Form from the previous school
  • The child’s passport size photographs (2)
